I am aware of the risks and hazards inherent to participation in physical exercise activities, athletic training, and combative sports, including without limitation, martial arts, boxing kickboxing, karate, wrestling, grappling, interactive/contact training with others, and sparring (the “Activities”), and that such risks and hazards include physical damage and injury to myself and my property, including even death. I hereby elect to voluntarily participate in such activities, knowing that I do so at my own risk and knowing that they may be hazardous to me.

I have been advised to consult a physician prior to undertaking this or any exercise, physical training, or fitness program, or making any changes to my diet or nutrition plan, and I understand that it is my responsibility to do so. To the best of my knowledge, I am in good health and physical condition and fully able to participate in the Activities, which may involve strenuous exercise and rigorous physical exertion. Further, I understand that it is my responsibility to convey any injuries, physical limitations, or concerns I may have, whether of a temporary or ongoing nature, to the staff and students of Arcaro Boxing, LLC with whom I am training and that I should feel comfortable not engaging or discontinuing any activity - and am expected to do so - if I am concerned about the risk involved.

I permit the instructor(s) of Arcaro Boxing, LLC to secure needed emergency care in case of an emergency when I cannot be contacted. I understand that any information I have provided will remain confidential within the Arcaro Boxing LLC program.

By signing up for classes or events, the attendee grants Arcaro Boxing, LLC permission to use his/her likeness in a photograph and/or video in any and all publications and materials without payment or consideration made to them. The attendee realizes these photos and/or videos become the property of Arcaro Boxing, LLC and will not be returned. The attendee authorizes Arcaro Boxing, LLC to use, edit, copy, publish or exhibit any photo or video for any lawful purpose. The attendee waives the right to review any photo or video or to obtain royalties from the photo or video.

On behalf of myself, members of my family, my heirs, executors, administrators, successors and assigns, I hereby now and forever release, discharge and hold harmless Arcaro Boxing, LLC and all its past, present, and future members, managers, owners, officers, employees, agents, and representatives (the “Released Parties”), now and forever, for any injury, loss, or damage to my person or property occurring at Arcaro Boxing, LLC or arising out of or in connection with my participation in the Activities, howsoever caused, to the fullest extent allowable under applicable law and notwithstanding that such injury, loss, or damage may have been contributed to or occasioned by the negligence of any student, guest, or invitee of Arcaro Boxing, LLC or any Released Party. I acknowledge that I have carefully read this Waiver and Release document and fully understand that it is a release of liability and I am agreeing voluntarily to waive any and all right I may otherwise have to bring a legal action against Released Parties for personal injury or property damage.

By checking here, you are consenting to the use of your electronic signature in lieu of an original signature on paper. You have the right to request that you sign a paper copy instead. By checking here, you are waiving that right. After consent, you may, upon written request to us, obtain a paper copy of an electronic record. No fee will be charged for such copy and no special hardware or software is required to view it. Your agreement to use an electronic signature with us for any documents will continue until such time as you notify us in writing that you no longer wish to use an electronic signature. There is no penalty for withdrawing your consent. You should always make sure that we have a current email address in order to contact you regarding any changes, if necessary.
